All visits are by appointment only. Although we try to stay on schedule as much as possible, delays do occasionally occur. We appreciate your patience. Please assist us by arriving for your appointment on time. Due to scheduling constraints, if you arrive greater than 15 minutes late for your appointment time, we will need to ask you to reschedule.

Although your appointment is especially reserved for you, we understand that circumstances can occur that can conflict with your scheduled time. We request 24 hours notice in the event that you need to cancel or reschedule an existing appointment.

At your initial visit with our office, you will be asked to complete some registration paperwork, including a medical history. A link to these forms is provided below if you prefer to complete these in advance of your visit. In addition to the registration forms, we will need a copy of your current insurance card and photo I.D. for our records. Please be aware that we cannot file an insurance claim without a copy of your current insurance card.
